Below are all of the documents you will need for the Grant Program process from start to finish. Please note that this year, applications are being accepted online only (link provided below).
Please familiarize yourself with the Grant Guidelines before submitting the Grant Application. Once your group has been awarded a grant, grantees must complete a Post-Grant Summary and submit a Check Request for reimbursement (if you do not have an invoice from a vendor). Funds must be used within 12 months of receiving award. Checks will not be released without first receiving a check request, including all original receipts attached. All grant materials must have proper signatures.
